Pneumococcal Vaccine in Adults: A Quick NP Guide
Pneumococcal disease can cause pneumonia, meningitis, and sepsis — especially in older or immunocompromised patients. As a new nurse practitioner in primary care, you’ll need to know which pneumococcal vaccine to give, when to give it, and how to explain the options to patients.
Who Needs Pneumococcal Vaccination?

All adults ≥65 years old (regardless of health status).

Adults 19–64 with risk factors, including:

Immunocompromised (HIV, cancer therapy, transplant, asplenia).

Chronic conditions (diabetes, heart disease, chronic lung disease, liver disease).

Smokers and heavy alcohol use.

Vaccine Options & Schedules
PCV20 (Prevnar 20)

Single dose, no further pneumococcal vaccines needed.

Easy, one-and-done option for most patients.
PCV15 (Prevnar 15)

If given, must be followed by PPSV23 (Pneumovax 23) at least 1 year later (minimum 8 weeks if immunocompromised).
PPSV23 alone?

No longer recommended as the sole vaccine in adults. Always use PCV first if starting new.
📌 Clinical Pearl: For most adults, PCV20 is simplest — especially in primary care. PCV15 + PPSV23 is still an option if your clinic or insurance formulary requires it.
Revaccination Rules

If a patient already had PPSV23 before 65, give another dose at ≥65 (at least 5 years after the last one).

If they’ve already had PCV13 in the past, treat as though they had PCV15 — still give PPSV23 if indicated.
